In addition to your payment, if you haven't alreday, please:

1) Complete this registration form, 

2) email a copy of your puppy's vaccination record (DAPPV) to [email protected]*, and

3) attend our online Orientation class.

*) You can also bring your puppy's vaccination record to your first class, but you must not forget. Without both the registration form and the vaccination record, we must unfortunately send you home again, as we need these documents on file and in order to protect the other puppies in class.
